easons rates are separated into three levels.  This structure helps determine the amount of care required to meet the needs and comfort of individuals as time progresses.  Thus, it is of great importance that we appropriately place residents in a compatible environment with specialized activity programs geared to their capabilities.  Alzheimer's does not affect everyone the same way or in the same manner, so adjustments from one level to another will be dependent on the individual needs.

Not all qualifying behaviors may be present, but one or more exhibited behaviors may be sufficient to warrant a move to the next level to ensure the individual's comfort. 

Our staff is specially trained in the level of care they provide.  Placement decisions involve a multi-disciplinary team which consists of the Administrator, Director of Nursing, Social Services Director, and our Medical Director.  The following guide will help you understand the levels of care and criteria we use at Seasons Care Center.

Please note that this guide is not conclusive and should be used only as a general outline of conditions.  Careful consideration will be on an individual basis.

Level 1 Rates Criteria for Level 1 Community
Semi-private
Private
Respite
Medicaid Certified
-Ambulatory, may use walker or cane
-Feeds self with or without verbal cues
-Requires minimal assistance with daily living activities
-Difficulty with "word finding"
-Participates in activities of choice
-Turns self in bed
-Occasional incontinence of bowel and bladder
-Occasional behavioral difficulties
-Re-directs easily
 
Level 2 Rates Criteria for Level 2 Community
Semi-private
Private
Respite Care
Medicaid Certified
-Unsteady gait, may use walker or wheelchair
-Needs much verbal assistance to eat, may need staff feeding
-Decreased level of functioning-needs more tactile stimulation activities
-Increased incontinence of bowel and bladder
-Requires moderate assistance with daily activities
-Increased behavioral difficulties
-Needs assistance with turning in bed
-Exhibits incomplete sentences and thoughts
-May exhibit swallowing difficulties
-Requires more passive activities
-May be candidate for Restorative Aide program
 
Level 3 Rates Criteria for Level 3 Community
Semi-private
Private
Respite Care
Medicaid Certified
-May be bedridden or in Geri-chair
-Non-communicative, may yell out
-May require specialized equipment
-Fed by staff and or enteral feeding required
-Limited participation in activities
-Incontinence of bowel and bladder
-Must be turned and repositioned in bed
-Requires Restorative Aide program
-Requires more in-depth assessment of overall nutritional needs

Ancillary expenses will be billed separately.
A processing fee of $50 will be assessed with all new admissions.

Bed Reserves are required and charged at the normal daily rate upon availability prior to admission date.  Bed reserves are also required when a resident takes a leave of absence from the facility or is admitted to a hospital.

No one shall be denied admission on the basis of sex, creed, color, race, national origin, age, handicap or communicable disease.
