Admission Criteria
 |
Must have a supporting diagnosis of
Alzheimer's or other related disorder
|

Must have a physician's order for skilled
nursing care.
|
 |
Must exhibit a need for a secure environment due to
safety.
|
 |
Must not be a threat or harm to self or others.
|
 |
Must have a primary physician who makes nursing home
visits or have a caregiver who will take the individual to the physician
on a monthly or as-needed basis.
|
 |
Must have a "Medical Durable Power of Attorney,"
guardianship or "Power of Attorney" prior to admission
|
 |
Must have private long-term care insurance, Medicaid or be private
pay status.
|
 |
Must have a complete current History and Physical prepared
by a primary physician.
|
What to Bring
 |
Comfortable loose-fitting pajamas or other sleepwear.
|
 |
Consider easy-on and easy-off clothing. Some
residents may require adaptive clothing.
|
 |
Appropriate memorabilia.
|
 |
Appropriate well-fitting footwear. Tennis
shoes, house slippers or rubber-soled shoes are excellent.
|
 |
Furniture (if providing a recliner chair, please consider
one with plastic or vinyl covering)
|
 |
Personal items such as dentures, glasses, hearing aides (please have
resident's name engraved or made clearly identified).
|
What Not to Bring
Due to the
compromised memory level of an individual with Alzheimer's, we have
compiled a list of items to leave at home.
 |
Personal items of large monetary or extreme sentimental value that may
be irreplaceable.
|
 |
Over-the-counter medications, creams, after-shave
lotion, perfume or home remedies. These tend to be mistaken as
edible.
|
 |
Personal prescription medications. Our pharmacy
provides our medications in the appropriate dispensing devices.
|
 |
Large furniture, throw rugs, or other large objects
of art. Our rooms are quite spacious, however, a crowded living area
can lead to potential slips and falls. Please consider size when
choosing furniture and decorative items.

 |
Expensive jewelry. Costume jewelry works well and makes your loved
one feel just as decorated
|
